Returning Players

Registration materials are mailed after April 1st to the address of record. If you moved since last season you will need to attend a walk-in registration.
Parent/guardian needs to:
1. Sign membership form
2. Parent/caretaker agreement form
3. Special request form (if applicable).
Return the above, plus:
4. Copy of birth certificate - returning players don't need to provide a copy, this only applies to new players.
5. If playing U9-U14, A small photo (1” x 1”) – please write name of child on back of picture.
Mail your completed registration packet (membership, parent/caretaker and special request form), check or money order made out to EGYSL, birth certificate and photo to the club address.

Registration fees 2010

Registration fees from April 1, 2010 to May 31, 2010
$125 for the 1st child
$125 for the 2nd child
$100 for 3rd child
$80 for 4th child
$50 for 5th child
Late Registration fees from June 1, 2010 to September 1, 2010 (Space Available only)
$145 for the 1st child $145 for the 2nd child
$120 for 3rd child
$100 for 4th child
$50 for 5th child
Refund Policy
Prior to June 1st: Full Refund
June 1st to September 1st: 50% Refund
After September 1st: No Refund
Season Registration Information

Players must be at least 4 years old by July 31st of the year of registration in order to be eligible to play in the EGYSL. Regular registration runs from April 1 to May 31, Late registration is from June 1 to Sept 1 on a space available basis only.
We require one copy of either a county/state birth certificate or other acceptable proof of age. Approved proof of age documents include government issued birth certificate, alien registration, passport, or DMV Identification card. NO BAPTISMAL/HOSPITAL OR RELIGIOUS CERTIFICATES ACCEPTED.
