Season Registration Information
Players must be at least 5 years old by July 31st of the year of registration in order to be eligible to play in the EGYSL. The only exception is if the child is enrolled in Kindergarten for the upcoming school year. Regular Registration runs from April 1st to May 31st.
Registration fees from April 1, 2008 to May 31, 2008
$125 for the 1st and 2nd child
$100 for 3rd child
$80 for 4th child
$30 for 5th child

Registration
materials are mailed after April 1st to the address of
record. If you moved since last season you will need to
attend a walk-in registration.
Parent/guardian needs to:
1.
Sign membership form
2. Parent/caretaker agreement form
3. Special request form (if applicable).
Return the above,
plus:
4. Copy of birth certificate - please provide a copy
even if your child was registered last year.
5. If playing U9-U14, A small photo (1” x 1”) – please
write name of child on back of picture.
Mail your
completed registration packet (membership,
parent/caretaker and special request form), check or
money order made out to EGYSL, birth certificate and
photo to the club address.

Incomplete
registrations will not be processed until ALL
information required is received and may be returned to
the parent/guardian.
